Initial details reprinted from book entitled "From Heaven To Hell" with permission of author Ben LeLandais and extended or corrected through research of Dick O'Donnell.  Note that some names are duplicated in two or even three aircraft and are annotated as such.  Author LeLandais has no explanation other than the fact that the lists he received contained these errors.
